The purpose of the HSSP Sixth Semester Academic Information Roster is to obtain critical academic data, which is available the third term prior to graduation (typically the sixth semester).  Counselors are provided the opportunity to create a sixth semester roster for his/her high school rather than respond to an ISAC-generated roster.

Required Data

High Schools must provide the following data, in the order listed below, for those students in the top 50% of the class at the end of the third semester prior to graduation (normally the sixth semester) on the HSSP 6th Semester Academic Information Roster. Be sure that only sixth semester data is submitted.

All school-generated HSSP rosters must include the following three items:

  • Student Name - last name, first name, middle initial (if available) listed alphabetically by last name; and
  • Student date of birth; and
  • All schools (both ranking and non-ranking) must provide unweighted GPAs. 

In addition, all school-generated HSSP rosters must include one of the following: 

  • Class rank for the top 50% of students; or
  • Cumulative unweighted Grade Point Average (GPA) for those students in the top 50% of the class.

Attach a cover letter to your school-generated roster that includes a contact name for questions, a signature of the submitting party, and the following:

  • high school's six-digit ACT code;
  • class size at the end of the sixth semester; and
  • GPA scale (4.0, 5.0, or other).

Transfer Students

For those students who transferred to another high school after they completed their sixth semester at your high school, indicate on the roster their class rank and unweighted GPA as of the end of their sixth semester at your school, and provide the name and address of the new high school they are attending, as well as the date they transferred.

Disqualification Codes

If you know a student is not eligible for an HSSP award, please make a notation to that effect on the school-generated roster.  The codes and the corresponding reasons for disqualification are as follows:

  • E - not a citizen or eligible non-citizen;
  • F - not a resident of Illinois as of June 30, 2012 (provide updated address);
  • H - not scheduled to graduate from high school by September 1, 2013;
  • I   - already in college or will graduate from high school prior to January 1, 2013; or
  • O - deceased.
