Procedures
  1. Complete the Court of Claims complaint form in its entirety; include student(s) Social Security Number and your Federal Employee Identification Number.
  2. If an attorney represents you, complete the appropriate section of the complaint form so correspondence may be directed to the attorney's office.  An attorney is not required in order to file a lapsed appropriation claim.
  3. Sign both claimant lines of the complaint form, and have the signatures notarized.
  4. When submitting claims, include only students who are to be paid for the same fiscal year and ISAC program (e.g., MAP, Illinois National Guard, etc.) on each form. For One Student - complete the Court of Claims complaint form (notarized), plus see Procedure #8.  Be sure to provide the student-specific information ISAC needs to process the claim.

    For Multiple Students - complete the Court of Claims complaint form (notarized) and the Court of Claims Multiple Student Request Form, (note: the columns on the form indicate the student-specific information ISAC needs to process the claim), plus see Procedure #8

  5. As of July 1, 2000, there is a two-year limit to file Court of Claims (705 ILCS 505).
  6. No filing fee is required for lapsed appropriation claims.
  7. Collate the Court of Claims complaint form, the Court of Claims Multiple Student Request Form, if needed, along with any itemized bills, invoices, or other materials that substantiate your claim.  Make five copies of all forms and supporting documentation, attach together, and mail (original plus five copies of everything) to: Ellen Schanzle-Haskins

  8. The Court of Claims will forward this claim to the Illinois Student Assistance Commission (ISAC).  To expedite ISAC's servicing the claim, please include a letter explaining why a student(s) payment was not requested during the academic year.  Include additional documentation if it is necessary to verify the student(s) is in compliance with all Monetary Award Program (MAP) requirements and/or all other gift assistance programs.
