STS Participation and Reporting
{ISAC Rules, Section 2770.40}

Schools participating in the STS Program of Matching Grants are required by ISAC Rules to complete and submit specific forms to ISAC by August 15th. Each institution desiring to participate in the STS Program must complete the Participation Information Update Form by August 15th preceding each award year and an annual Performance Report must also be submitted no later than August 15th following each award year.

Participation Information Update Form

The Participation Information Update Form is completed to indicate your institution's intentions to participate in the STS Grant Program during the next two academic years. The estimates provided will assist ISAC in planning financial needs for the 2009-10 academic year and in preparing ISAC's budget request recommendation for funding for the 2010-11 appropriation.

Performance Report

Information provided on the Performance Report summarizes awards made to students and funds used by your institution. This information is provided to the Illinois Board of Higher Education and will be audited. Keep a copy of this document for your records.

How to Complete the Documents

The documents have been created in Microsoft Word 2000 and Adobe Portable Document Format (PDF) for your convenience.

Using the Word Document

The Microsoft Word document can be saved to your computer so that you can complete it electronically using Microsoft Word. (It can also be saved in other popular word-processing programs, including all versions of WordPerfect.) The completed document can then be returned to ISAC as an e-mail attachment from the Financial Aid Director.

If you are using an older version of Microsoft Word, you may not be able to open or save the Word document and will need to use the Adobe PDF file. Also keep in mind that printer settings vary among computers and could cause the document to appear and print differently from one computer to the next.

  • Using the PDF File

    Information cannot be added to the Adobe PDF file electronically; instead, it must be printed and completed by hand, and then returned to ISAC via fax or mail. The Adobe PDF file can be saved to your computer for future use or reference. Another option to consider when using the Adobe PDF file is to complete the document, scan the completed pages and return it as an e-mail attachment from the Financial Aid Director.

    Adobe Acrobat Reader is required to access the Adobe PDF files. If you have trouble opening the file, you may need to download a newer version of the software. Adobe Acrobat Reader is a free and freely distributable software that can be downloaded or upgraded through Acrobat’s Web site.

    Either format can be submitted but must be returned to ISAC on or before August 15th, by mail or fax or as an e-mail attachment to the address listed below. Be sure your institution’s name appears on both documents.
