At the end of each academic year, the Optometric Education Scholarship Program (OESP) Enrollment Status Verification for Compliance Roster is e-mailed to the postsecondary school from ISAC, along with a cover letter explaining what is needed and the due date for providing the information.The institution verifies that the individuals who were previously awarded OESP were enrolled during that academic year.
Enter one of the enrollment verification codes listed below and a status date for each recipient. A status code must be included for all recipients except "I" and “C”:

A Optometric Education Scholarship Program (OESP) Enrollment Status Verification Form must be completed and returned to ISAC with the OESP Program Enrollment Status Verification for Compliance Roster.
