Application
The 2009-10 Optometric Education Scholarship Program Application is now available. Students may obtain the application from the Financial Aid Office at the Illinois College of Optometry. In order to receive priority consideration for the award, the completed application should be submitted on or before March 1, 2009.
2008-09
Enrollment Status Verification for Compliance
The 2008-09 Enrollment Status Verification for Compliance roster and Enrollment Status Verification Form were e-mailed to the postsecondary school on August 11, 2009. Verification of the enrollment status is requested for the Optometric Education Scholarship Program (OESP) recipients for whom our records indicate are enrolled at your school. Verification of enrollment is needed to meet compliance requirements for the 2008-09 academic year. The completed Verification for Compliance roster and the signed and dated Verification form must be returned to ISAC on or before October 30, 2009.
